Very poor discharge planning relative being sent back to Apt. Without notifying family out of State and not appropriate level of care and then when trying to speak with someone at the Administrative level about the discharge was told that no one at Administrative level was available in essence no one was in charge but I could leave a message and someone could call me in the morning. I HAVE worked in Healthcare specifically Nursing Home Discharge planning in N.Y. for many years this is totally inappropriate and placing the patient my relative at risk. N.Y. has DOH complaint reporting and this clearly aappears to be a reportable incident.
